Media Release Form

I hereby give my consent and authorize 15 Minutes With Dad Podcast to use, reproduce, distribute, and broadcast my name, voice, likeness, image, and any other material provided by me or recorded during the interview, for any editorial, promotional, or commercial purpose related to the media organization, including but not limited to print, radio, television, online, social media, and other electronic and digital platforms.

I acknowledge that 15 Minutes With Dad Podcast owns all rights, title, and interest in and to the interview and any materials provided by me or recorded during the interview, and I waive any claims to any ownership or compensation for such use.

I also acknowledge that 15 Minutes With Dad Podcast has the right to edit, alter, or modify the interview or any material provided by me or recorded during the interview as it deems appropriate, without my prior approval, and I release [insert name of media organization] from any liability that may arise from such editing, alteration, or modification.

I represent and warrant that I have the right to grant this consent and authorization, and that my participation in the interview and the use of my name, voice, likeness, image, and any other material provided by me or recorded during the interview, do not infringe, or violate any third-party rights or any applicable law or regulation.

I agree to indemnify, defend, and hold harmless 15 Minutes With Dad Podcast, its affiliates, officers, directors, employees, agents, and assigns, from and against any and all claims, damages, liabilities, costs, and expenses (including reasonable attorneys' fees) arising out of or in connection with any breach of my representations and warranties, or any claim or dispute related to the interview or the use of my name, voice, likeness, image, or any other material provided by me or recorded during the interview.

I have read and understand this media release form, and I freely and voluntarily sign it to bind myself, my heirs, executors, administrators and assignees.
